Explaining ADHD to someone who doesn’t live with it can be challenging, as there is a good amount of misunderstanding about it. It is much more nuanced than an inability to pay attention or sit still. If you are trying to help family, friends, or colleagues understand what is actually going on for you, this overview may help.
The Official Version
Officially and medically, Attention Deficit Hyperactivity Disorder is classified as a neurodevelopmental condition in the Diagnostic and Statistical Manual of Mental Disorders (DSM-5). It is characterized by persistent patterns of inattention, hyperactivity, and impulsivity that interfere with a person’s ability to organise, focus, and function in daily life.
Developmental conditions originate during the brain’s growth phase in childhood. The ADHD brain grows and builds its circuitry differently from the standard average. ADHD is not a behavioral disorder, a character flaw, or a learning disability.
My Preferred Version
Rather than viewing it as a deficit, I consider ADHD to be a natural variation in brain wiring, creating a distinct neurodivergent profile or neurotype. This difference shows up as a variation in how we regulate and focus our attention, rather than a lack of it.
An ADHD brain does not actually lack attention. Instead, it struggles to direct that attention on command, especially toward tasks that lack intrinsic interest. However, when a person with ADHD engages with a topic that is deeply interesting, high-stakes, or urgent, the brain turns on completely, allowing them to lock in and focus with immense depth.
The Biological Mechanisms
To understand why this happens, it helps to look at four key biological mechanisms:
1. Brain Chemistry (Dopamine and Norepinephrine)
The ADHD brain operates with lower baseline availability or faster clearance of two essential chemical messengers:
- Dopamine: Responsible for motivation, task activation, and satisfaction. Without steady dopamine, the brain struggles to assign value or urgency to routine, non-novel tasks.
- Norepinephrine: Responsible for alertness, prioritisation, and filtering out background noise. Lower levels make it harder to keep track of multi-step instructions or ignore irrelevant distractions.
2. The Internal “Switch” (The Default Mode Network)
In a standard brain, when you engage in a task, the brain’s mind-wandering circuit (the Default Mode Network, or DMN) automatically turns off so the task-focused circuit can take over.
In an ADHD brain, that switch is less definite. The DMN stays active in the background even while you are trying to work. Because the DMN is the exact network we tap into for imagination, daydreaming, and creative visualization, the mind-wandering and task-focused networks compete for bandwidth simultaneously. This creates a lot of internal noise, but it is also the source of deep creativity for people with ADHD.
3. A Different Growth Timeline
Brain imaging shows that the prefrontal cortex, the brain’s management centre, matures on a delayed, non-linear timeline compared to sensory and motor regions. While it reaches full maturity later in adulthood, this structural mismatch means the brain’s long-term planning pathways are under-resourced compared to its immediate sensory drives.
4. Genetic Wiring
ADHD is highly heritable (around 70% to 80%), but it is rarely caused by a single gene. Instead, it is polygenic, meaning thousands of small genetic variations act together to shape how neural pathways form, prune, and transport neurochemicals. It is a natural spectrum of human processing rather than a localised defect. I think of it like the tapestry of our brain is woven slightly more creatively!

Executive Functioning and Daily Impact
The prefrontal cortex manages executive functioning, which is our ability to plan, prioritise, track time, regulate emotions, and execute complex projects. Because the modern world is designed for standard brains, these differences create genuine barriers for people with ADHD. While ADHD is not a disorder, it can create disorder in our lives!
Living with ADHD requires an enormous amount of unseen effort. Simple, linear tasks can consume twice the cognitive energy of a neurotypical peer, leading to executive function fatigue and burnout.
To keep up with the demands of work, study, and home life, people with ADHD often have to over-function, resulting in anxiety, overwhelm, and exhaustion. Many also experience Rejection Sensitive Dysphoria (RSD), an intense emotional vulnerability to criticism or perceived failure, alongside the fatigue that comes from “masking” their traits to fit in.
The Unique Strengths
The unique wiring of ADHD brains also brings distinct cognitive strengths:
- Lateral thinking and imagination: out-of-the-box problem solving and creative insight.
- Hyper-focus: The ability to enter a state of deep, fast-working flow when a task aligns with a genuine interest.
- Adaptability and resourcefulness: Natural resilience, calm in a crisis, and a capacity to pivot quickly when circumstances change.
- Empathy and sensitivity: Heightened sensory and emotional awareness that fosters strong intuition and connection with others.
ADHD in Australia
- Prevalence: It is estimated that 2.5% to 5% of adults and 6% to 8% of children live with ADHD in Australia, which translates to roughly 800,000 to over 1 million people. These percentages are very similar to worldwide prevalence.
- The Diagnostic Gap: Historically, diagnostic criteria focused on hyperactive young boys. Consequently, millions of girls, women, and adults with the quieter, inattentive presentation went completely unnoticed.
